Patient Responsibilities
Copays are due at the time of service. There is a $25.00 administrative fee for processing your copay after your visit. It is your responsibility to advise the front desk of any changes in your health insurance, address or phone number at the time of your visit. Please make sure your children are not eligible for another primary insurance from a former spouse or father of the child. If it becomes necessary to rebill any claims to another insurance at a later date, you will be charged a $35.00 rebilling fee per claim. This charge is not covered by your insurance and will be your responsibility.
